Academic departments or professors who are interested in hosting international faculty members should fill out the following forms and contact GEO about the obligations and privileges inherent in hosting a foreign faculty member. 

Please be sure to fill out each form completely and accurately. The Global Education Office will be unable to issue the State Department form DS-2019, required for issuance of a J-1 visa, without all necessary information filled out on each form and the required signatures.
