Please be aware of the following form deadlines. Most of these forms are, or will soon be, available on the Hub at thehub.hampshire.edu. You will also need to complete the Health Services forms, which were sent to your home address in early May.

Students with disabilities and international students may have additional forms to complete. Please visit disabiities services and/or new international students for more information.
