Patient Portal

Hampshire Health and Counseling uses a password-protected online patient portal to communicate with students about test results and follow up, send patient education materials, and ask administrative questions related to health care. We do not use email to communicate about medical or mental health concerns or any protected health information because it is not secure enough to protect patient privacy.

The portal should never be used to ask urgent questions or address medical or mental health problems. Please call Hampshire Health and Counseling at 413.559.5458 with any medical or mental health concerns. The office is open Monday–Friday, 9:00 a.m.– 4:30 p.m. when school is in session. Additionally, there is after-hours phone support available for mental health concerns and medical triage nursing advice.

The portal can be used to:

  • Ask general questions.
  • Submit required health forms, immunizations, and tuberculosis screenings.
  • Access and update your immunization records.
  • Obtain helpful handouts.
  • Provide us with copies of notes, prescriptions, labs, and other health records.

Please note: Clinicians and office staff do not receive or respond to secure messages when Hampshire Health and Counseling is closed, including after hours and weekends. Additionally, clinician schedules may mean that messages are not seen for one or two days during the work week. If you have an urgent or same-day issue, please call the office at 413.559.5458.

Access the Patient Portal

Hampshire Health and Counseling is a satellite clinic of the UMass University Health Services, so the portal is a University of Massachusetts at Amherst website.
